Pediatrics Plus has a dedicated Family Relations Team to help you navigate everything from insurance to physician referrals. Here’s an overview of the process and tips on how you can help expedite the process.
1. Getting Started
You can begin the enrollment process 2 ways:
- Referral by your child’s physician
- Complete the Pediatrics Plus Enrollment Form
Did you Know?
Prescription rules vary by state. Depending on your payor, you may need two “prescriptions” or “referrals” from your child’s physician through the enrollment process: one is for the evaluation (to determine your child’s needs) and the other is to sign-off on the therapy treatment plan (to begin therapy). You can help expedite the process by ensuring your child’s physician provides these important documents to Pediatrics Plus in a timely manner.
2. Family Relations is here to help!
After we received the physician referral or your enrollment form, a Family Relations Specialist will contact you to complete the intake process which includes:
- Completion of the Enrollment Form.
- If we haven’t already received it, Pediatrics Plus will send a request for a prescription evaluation/referral to your child’s physician. Please note: If your child already has a previous evaluation or diagnostic report, please share it at this time – it will save you a step!
- Gather insurance information and verify funding source.
Funding sources and timelines to secure those resources may vary. Your Family Relations Specialist is here to help guide you through this process.
- Complete pre-visit “Welcome Packet” that includes information such as medical history, consent forms, etc. You can choose to receive this information via text or email during your call with Family Relations. The email will come from

3. Evaluations
Once all prescriptions/referrals are received from your child’s physician and the above paperwork is completed, your Family Relations Specialist will call to schedule your child’s evaluation.
After the evaluation is completed, your therapist will call with the results within 48 business hours.
If therapy needs are determined necessary, a treatment plan will be sent to your child’s physician for approval.
4. Therapy Begins!
Once approved, someone from your selected Pediatrics Plus location will call you to schedule treatment!
